Lakewood Nursing and Rehabilitation Center is dedicated to the promotion of health and the advancement of growth for residents, personnel, and the members of the community. We believe in the dignity of the human person. We recognize that each perseon has physical, emotional, social and spiritual needs and rights. We are committed to meeting these needs and ensuring the preservation of all rights.

Admission to the facility is based on the ability to meet the residents need for care and provision of an environment most appropriate for those residents. It is the policy of Lakewood Nursing and Rehabilitation Center to admit and treat all residents without regard to race, color, creed, national origin, handicap, religion, gender or age. No distinction in eligibility for, or in the manner of providing, resident services is made on these basis. The facility reserves the right, however, to refuse admission to any person who does not meet admission requirements.

The person designated to coordinate compliance with Section 504 of the Rehabilitation Act of 1973 (non-discrimination against the handicapped) is the Administrator of the nursing home.

Each Resident admitted to the facility must provide a signed physician's order for care as well as complete the facility's admission care plan preference form, DNR form, and admission application.

Pursuant to federal law, the nursing home will not admit any resident who is mentally ill, any persons under 18 years of age or any mentally retarded individual.
